Section I – Define query population (upper left panel)
Step 1: Choose geographic area
Use the three radio buttons to select an ESRD population from the entire U.S., a selected ESRD network, or specific states/territories. Clicking on “Selected ESRD Networks” will default to network 1; click on the drop-down arrow to the right of the selection box to choose a different network from the drop-down list. You can select only one network. Similarly, clicking on “Selected States/Territories” will default to Alabama; choose a different state/territory from the drop-down list. You can select only one state or territory. Hover your mouse over the criteria title for more details.
Step 2: Choose a range of data years
Use the slider bar to specify the range of years for your query. The default selection is the most recent five years. Drag the round button left and right to change the start and end year for your query population.
Step 3: Choose age
Use the five radio buttons to choose patient age ranges for your query. By default the range is all ages. Click on “0-17” to choose the pediatric population, or click “18+” or “65+.” Use the last radio button to choose a specific age group. Click on the drop-down arrow and choose from the drop-down list. You can choose one or multiple age groups, or click “Select All” to include all age groups and click “Deselect All” to clear your selection.
Step 4: Choose sex
Choose the sex of your query population using the three checkboxes. Default is both sexes and unknown.
Step 5: Choose race/ethnicity

Step 6: Choose treatment modality
Use the drop-down selection box to choose one or more treatment modalities for your query. Click “Select All” to select all treatment modalities or “Deselect All” to clear your choices. Default is all modalities.
Step 7: Choose primary ESRD diagnosis group

Step 8: Reset your selection (optional)
Click the reset button to clear your selections and set all criteria back to default.
Section II – Define query display option (lower left panel)
Step 1: Choose first display option
Click on a radio button to choose the first element with which to summarize your query results. For example, the default selection is year, and the resulting table will break down query results by years. Similarly, the resulting bar chart displays query results by year.
Step 2: Choose second display option
Click a different radio button to choose your second display option. The default selection is “None.” For example, if you click “Sex,” the query results will be displayed by both year and sex. The resulting table will show unique years in the first column, and sex categories across the top row. In the chart, the x-axis will be the first display option, and each bar the second display option. In this case, the x-axis is year, and bars show sex categories.
You can select up to two summary variables by which to display the data. No secondary display options are available when the first is state or network.
